  • Log In and Access Your Student’s Profile
Once logged in to FinalForms, go to your student athlete’s profile. In this case, let’s say we’re updating Kenny’s forms. Click on “Update Form” next to his profile.

  • Make Your Updates
In this example, we’ll pretend that Kenny was diagnosed with a new food allergy at a recent doctor’s appointment. Navigate to the “Past and Ongoing Health Conditions” section, where you can add or update medical information.

 

Select “Yes” for allergies, and then choose “Food Allergies.” In the notes section, you can add specific details such as “Tree nuts—must have EpiPen.” This ensures that the school, coaches, and staff have all the necessary information to keep Kenny safe in case of an emergency.


  • Sign and Submit the Form

Once you’ve made all necessary updates, scroll to the bottom of the page and sign the form. FinalForms timestamps your submission, giving both you and the school a record of exactly when the changes were made. This timestamp provides a clear audit trail, should any questions arise in the future.


  • Updating Other Information
If you need to make additional changes—such as updating emergency contact information—just scroll to the relevant field, make your changes, and then sign and submit again. Every change is recorded and timestamped, ensuring that your updates are always tracked.
